What is Cleft lip?
Cleft lip and palate is a congenital birth defect of face in which lip and palate are affected. There is vertical cut of lip and sometime palate as well. It result from non fusion of different parts of face and nose during development in 1st 4-8th week of pregnancy. If one side is involved it is called unilateral if both are involved it is called bilateral.
Causes of Cleft Lip:
There are multiple causes of cleft lip and palate. Both genetic and environment can cause it and most common causes are:
Alcohol
During pregnancy exposure to the anticonvulsant drugs like phonation is associated with a 10-fold increase in the incidence of cleft lip. Maternal smoking during pregnancy doubles the incidence of cleft lip. Other teratogens, such as alcohol, anticonvulsants, and retinoic acid, folic acid deficiency.
Incidence
Among the cleft lip and palate population, the most common diagnosis is cleft lip and palate (46%), followed by isolated cleft palate (33%) and isolated cleft lip (21 %). The majority of bilateral cleft lips (86%) and unilateral cleft lips (68%) are associated with a deft palate.
If the family has one affected child or parent with CLP, the risk of the child of the next pregnancy having CLP is 4%. If two previous children have CLP, the risk increases to 9%, and if one parent and one child were previously affected, the risk to children of subsequent pregnancies is 17%. For families with a child having CP, the risk of CP to children of subsequent pregnancies is 2%,6% if one parent has CP, and lS% if one parent and one previous child have CP.
Cleft Lift Examanation:

He would send you to a child specialist as well to properly evaluate other conditions like chest.
In 1st week of life Dr Usman Jahangir would advise you nasoalveolar molding (NAM)to bring the lip near to each other which improve surgical results. Alternatively lip tapping can also be done.
Cleft Lip Surgery:

Cleft Palate:
Cleft palate is associated with nasal regurgitation, recurrent infection, nasal speech and other speech. Dr. Usman Jahangir would perform palate surgery in Multan & Khanewal at age 10-12 month of child age.
Speech is assessed at age of 2-3 years and if speech problem persist surgery is performed at age of 4 years to improve speech. Similarly any abnormal lip scars, nothing is treated before child go to school. Dental treatment is started at 6-9 years of age and bone graft surgery is performed if there is any defect in alveolus. Or surgeries to improve facial shape (orthognathic surgery) are performed at puberty.
